Wireless Services Survey

I am conducting a nationwide (please, USA only) wireless survey. I would appreciate your input.

Please download the two-page MS-Word document containing 14 questions from:

(For some unknown reason, you'll need to manually copy the link into your brower's URL Address field. The link doesn't work automatically.)

No personal information (that would identify you) is asked, however, I do require that you email me your response using a valid email address. Your email address will not be given or sold to anyone and will be deleted from my records AFTER sending a confirmation-of-receipt to you. (If that confirmation comes back as an invalid email address, your submission will be disqualified from the stats.)
