It's a BB5 phone — to date the only confirmed way to unlock these is to get the unlock code from the provider that had the phone locked.
dmapr is correct. If the phone is new enough meaning 14 days old, try to take it back and buy it directly from Nokia. You'll pay a lot more but T-Mobile has so many features locked out on the 6133, it's pathetic. It is a good looking phone though. If all you care about is unlocking only, just tell them you have to leave for Canada in a few weeks and you'll need the phone over there for the Rogers or Fido network and most likely they'll get you the code but they may make you wait out the full 90 days after the purchase of the phone.
