How about Voicetones Studio at myPhoneFiles.com? You can make ringers straight from audio CDs, MP3, WAV, and OGG file or record directly from TV (connect TV audio-out to computer's audio-in) or record your own voice through MIC. There are even simple editing effects (to make ringtones louder for example, or fade the volume, etc.). It is in Beta so there may be some glitches but it works for the most part. I tested it with Sprint phones as well as Nokia 3650 or AT&T and T-Mobile networks. Other networks are a wild card at this point.
