Default Which phone, what service?

I have been overcome by the myriad of phones out there, so I have decided to seek advice from some of you experts out there. I am looking for a phone which has the following features:

Dual or Tri-mode (I tend to wander in the sticks so I need the analog)
3G or high speed enabled or next generation or 1xRTT, or whatever you call it
Contact list that can store not only numbers, but street addresses and e-mail addresses also
E-mail capable
Calender, alarms & reminders
Sync to PC nice, but not necessary
GPS services capable, maybe

As far as service is concerned, I am looking at TDMA or CDMA so I think either Verizon, or AT&T(T-Mobile or whatever they are calling themselves this month) will do. I live in South Bend (north central IN).

Any suggestions welcomed and appreciated.

You didn't say how many minutes you'd need so I'll just suggest a type of phone. It seems like you need alot of functions so why not go for a phone pocket PC. I know T-Mobile and Verizon offers a Pocket PC Phone. I don't think AT&T has one.
